2. Can anyone join?

Profile

Yes! Profile has plans for all types of people, whether you have diabetes, are looking to get pregnant or just want to feel better.

Calibrate

No, Calibrate is not for anyone who has a BMI of under 30 unless they have a metabolic condition. In that case, their BMI cannot be under 27. Those who are pregnant, breastfeeding, have an eating disorder, substance abuse issue, or cancer cannot join Calibrate. Nor can anyone over the age of 67.

Yes, no restrictions.

Form Health

No, you must have a BMI of 30 or 27+ with select medical conditions. Certain medical conditions are not compatible with the Form Health program. You also must be under the care of a primary care physician that you’ve seen at least once in the past year.

Profile

Yes, there is unlimited virtual 1:1 coaching included in your plan. Best part? These are REAL people. No prerecorded video's or bots.

Yes, fully digital plan. Members get 15 minute biweekly video sessions. A total of 26 1:1 coaching sessions per year.

The Mayo Clinic Diet

No, you can follow the Mayo Clinic Diet by reading the book, without any support. There are occasional virtual group video sessions with doctors that you can join in on.

Form Health

Yes, you will have a monthly meeting with a doctor and meet twice a month with an accountability coach.
